- Aspirate before injection; use fractionated dosing (3–5 mL increments).
- Treat seizures with midazolam; secure airway with 100% O₂.
- Lipid emulsion 20% — 1.5 mL/kg bolus, then 0.25 mL/kg/min infusion.
- Modified ACLS — epi ≤ 1 mcg/kg, avoid vasopressin, CCB, β-blocker.
